This form is used to document occurrences relating to health and safety issues (such as falls, medication errors, testing/procedural errors, adverse reactions, etc.), inappropriate care, employee health (including employee injuries), and equipment/facility problems.

This form should be completed by a supervisor or willing employee who witnessed a work-related injury. Please submit completed form to Richard Darnell in Administration and Abby VandenBrink in the Business Office.

Wellness Release Time provides all full-time, benefits-eligible, employees the opportunity to use 30 minutes of their regular work hours, up to 3 times per week, to exercise or participate in other physical fitness activities. After discussing this opportunity with your supervisor, please complete the Wellness Release Time Acknowledgement form and return to Abby VandenBrink.
